Licensed Practical Nurse

North Jersey Recovery Center is seeking an Addiction Medicine Nurse Care Manager (LPN) who is able to work effectively with clients, medical staff, and clinical staff to coordinate all aspects of care, including health screenings, addiction medicine/referrals and medication management. Candidates will be able to provide client-centered care under the direction of the APN's and Attending Physician to improve the overall organization of healthcare for clients. The successful Nurse Care Manager will have the opportunity to make an impact on the lives of others daily.
Responsibilities:
Perform client-centered and trauma-informed clinical and administrative duties within the nursing scope of practice. Clinical tasks such as collecting and preparing laboratory specimens, recording vital signs, rooming clients to be seen by medical providers, changing dressings. Acting as liaison between clients, APNs, support staff, and dispensing pharmacy. Assisting with prescription processing, refills, prior authorizations, and insurance issues. Administrative tasks such as scheduling appointments, communicating via telephone, and documenting care in the electronic medical record. Identify available medical referral resources for the client when leaving the facility. Complete appropriate documentation in medical records and comply with the scope of practice and federal, state, and departmental policies when sharing and/or documenting client care. Exercise sound professional judgment and seek assistance as necessary to effectively manage client behavior and care. Experience/Knowledge:
Should have a basic understanding of the disease model of addiction, the role of medications for addiction treatment, confidentiality, and provision of trauma-informed care. Should therefore be encouraged and supported to attend educational opportunities related to the care of persons with substance use disorders. Licensed Practical Nurse (preferred). Schedule:
